Motor Vehicle Title Forms

• Title Application MV- 1- Apply for a new title or replace a title using this form.
• Affidavit to Certify Immediate Family Relationship MV 16 - Verify the buyer and seller of a vehicle are related.
• Bill of Sale T-7 -Complete a bill of sale for each buyer and seller when selling/purchasing a vehicle.
• Limited Power of Attorney T-8 - Give permission to another person to complete title and tag paperwork in your name. Driver’s license (or copy) for both parties is required.

Motor Vehicle Military Exemptions

• Veteran's Affidavit for TAVT Exemption MV-30 - Apply for a tax exemption if you are a 100% service connected disabled veteran. Requires supporting documentation.
• Military Annual Ad Valorem Tax Exemption PT-471 - Apply for a tax exemption if you are non-resident, active military stationed in Georgia.
